In a recent thread, @Ed Webb mentioned having to pay for dental tratment whilst in Ireland because he had forotten to take his E111/EHIC card with him. EHIC is the new name for the E111 card.

Probably a good time to remind others that if you are going to a trip to an European Economic Area (EEA) country or Switzerland (but not The Channel Islands, including Guernsey, Alderney and Sark, The Isle of Man, Monaco, San Marino, The Vatican), you should apply for a FREE European Health Insurance Card (EHIC), which may give you access to reduced-cost medical treatment. See Apply for a free EHIC card - Healthcare abroad - NHS Choices. You should also carry it with you and check that it is still in date. it's not a replacement for travel insurance.

When you have paid for treatment (eg, forgotten card), you may be able to later claim back the cost of treatment by phoning 0044 191 218 1999 (Monday to Friday, 8am to 5pm).

There is some information from the European Health Insurance Card (EHIC) website link above which relates to forgotten or lost cards, if you need to apply for a new card, or if found that your card has expired when you are abroad in the EEA:

If you’ve forgotten to take your EHIC card with you, need to replace a lost of stolen card, call 0044 191 218 1999 and apply for a provisional replacement certificate (PRC) which will cover you for treatment until you get home. You may need to provide your National Indurance Number for this. The PRC is what you need to get if you find you need to apply for a new EHIC card when you are abroad in the EEA.

And they don’t seem to send you a replacement card come expiry time, so make sure you check the date well before you go away!
 
Good post @Cymro.
Saves me digging out the number to claim back my cash but more importantly highlights the simple oversight of your EHIC. Never assume nothing will happen to you, I've learnt my lesson.
