@FAC51 - if the above does not work, did you make any changes to your van before the problem started? We could not figure out why the reversing sensors on a T5 were doing this sort of thing until LED reversing lamps (bulbs) were removed and replaced with the originals - problem solved.

Can this piece even fail? the 2 smaller ones no longer light up (but tested the bulbs work in the indicator light). So I’m guessing a fuse somehow blew? Break and indicators work but the rest don’t. is it a fuse per bulb ?
