Lumpy Tickover on start

My van has been stand for 10 days. the diesel heater has been the only thing running on it.

This morning, -5c, I started it up and it was very lumpy. Coughing and spluttering. I also noticed a lot of smoke from the Eberspacher which wasn’t there yesterday. I hit the throttle and it all just went away. I then went on a 35 mile round trip and I started the diesel heater and the smoke had disappeared. The run was fine and the problem appears to have gone.

The tank was very low and the fill up warning had kicked in.

Thoughts?
 
Sounds like air has been pulled into the fuel lines due to a micro leak over the 10 days of engine inactivity.
If it only does it when the tank is nearly empty, and / or long standing periods it's an air leak.
