Over heating, water pump or sensor????

Hi does anybody have any ideas or experience with the following?
It appears that my coolant temp gets very high 110-120 degrees!!! (Have an OBD11 dongle As my van has no gauge)
Actual high temp warning has only come on once for a few seconds then went off.
radiator fan runs flat out even on short journeys, and continues to run after ignition off.
header tank feels pretty hot, no leaks or steam etc.
could this be water pump issue??
 
No faults appear to be logged, all though I’m unsure if it would need to be scanned with better equipment. I’ve had another thought that it maybe the DPF trying to regen??

How many miles has it got on the clock, how old and when, if ever, did it have a cam belt and water pump?
I ask because if it's due all that soon, just get it done and see if it cures it.
